Terms for using Connect Your Health

Purpose of the application
Connect Your Health helps patients retrieve payer claims information through Patient Access APIs and estimate deductible accumulation from Explanation of Benefit and claim adjudication data. Connect Your Health is not a health plan, provider, emergency service, or substitute for professional advice.
Authorization
You may use Connect Your Health only to access your own information or information for a patient you are legally authorized to represent. You must complete the payer authorization process before Connect Your Health can access patient data.
Deductible estimates
Deductible values displayed by Connect Your Health are estimates based on data returned by the payer. They may not match the official accumulator, member portal, explanation of benefits, or final payment determination from your health plan. Your health plan remains the source of truth for benefits, coverage, deductibles, out-of-pocket amounts, and claim payment decisions.
User responsibilities
You are responsible for using accurate account information, protecting your devices and credentials, reviewing the Privacy Statement, and promptly notifying Connect Your Health of suspected unauthorized use.
Availability and data limits
The application depends on payer APIs and the data those APIs return. Connect Your Health is not responsible for payer outages, delayed claims, incomplete EOB records, unavailable historical data, or changes to payer API behavior.
Privacy
Use of Connect Your Health is also governed by the Privacy Statement, which explains what information may be collected, how it is used, how it may be shared, and how to contact us.
Suspension or termination
Connect Your Health may suspend or terminate access if use appears fraudulent, unauthorized, unlawful, harmful to the service, or inconsistent with these Terms.
No warranties
Connect Your Health is provided as is and as available. To the maximum extent permitted by law, we disclaim warranties of accuracy, availability, fitness for a particular purpose, and non-infringement.
