Privacy statement

How Connect Your Health handles patient access data

Application purpose

Connect Your Health is designed to help patients access health plan claims information and estimate deductible accumulation from Explanation of Benefit and claim adjudication data returned by payer Patient Access APIs.

Information we collect with consent

If you authorize access through your health plan, Connect Your Health may collect patient identity information returned by the payer, coverage and benefit information, claims and encounter information, Explanation of Benefit records, claim service dates, claim status, provider or service descriptions, and adjudication amounts such as deductible, copay, coinsurance, allowed, and paid amounts.

Information we do not need

Connect Your Health does not need access to your contacts, photos, camera, microphone, precise device location, advertising identifiers, or social media accounts to provide deductible and claims review features.

How we use information

We use patient access data to display claims information, estimate deductible accumulation, troubleshoot support requests, protect the security of the application, and comply with applicable legal obligations. We do not sell patient health information.

Sharing

We do not share patient health information with advertisers or data brokers. We may share information with service providers that help operate the application, but only as needed to provide, secure, or support the service. We may also disclose information if required by law.

Storage, security, and retention

Connect Your Health will use reasonable administrative, technical, and physical safeguards designed to protect patient information, including HTTPS, access controls, encrypted storage where applicable, and audit logging for production systems. Patient access data will be retained only as long as needed to provide the service, satisfy legal obligations, resolve disputes, or enforce agreements.

Your choices and deletion

You may choose not to connect your health plan. You may stop using Connect Your Health at any time, request deletion of information held by Connect Your Health, or revoke payer authorization where your health plan makes that option available. To request deletion, email privacy@connectyourhealth.org.

Access and correction

If you believe information shown in Connect Your Health is inaccurate, contact privacy@connectyourhealth.org. Because claims data is received from your health plan, you may also need to contact your health plan to correct the source data.

Complaints

You may submit privacy or security complaints to privacy@connectyourhealth.org. You may also have the right to contact your health plan or applicable federal or state regulators.

HIPAA and third-party applications

Health information is sensitive. Once information is shared with a third-party application at your direction, different privacy laws may apply than those that apply to your health plan. Review this Privacy Statement and the Terms of Use before authorizing access.

Changes to this statement

We may update this Privacy Statement as the application changes. Material changes will be posted on this page with an updated effective date.
